Perception of the specialists in physical medicine and rehabilitation in Colombia on the usefulness of musculoskeletal ultrasound in clinical practice

Abstract

Introduction. Musculoskeletal ultrasound has become a fundamental tool for the daily work of specialists in physical medicine and rehabilitation around the world. Despite this, in Colombia the perception of these health professionals about the usefulness, safety and viability of us or in the country is unknown.

Objectives. To determine the perception of specialists in physical medicine and rehabilitation in Colombia about the usefulness and viability of musculoskeletal ultrasound in clinical practice.

Methods. Descriptive cross-sectional study conducted between March and September 2023 with 155 specialists in physical medicine and rehabilitation who practiced in Colombia. A virtual questionnaire was applied that included 27 questions: 12 on the characteristics of the specialists and 15 on the perception of the usefulness of musculoskeletal ultrasound. Associations between the perception of the use of ultrasound in musculoskeletal pathologies and the demographic characteristics of the physicians surveyed were explored.

Results. The median age of the participants was 47 years and the median time from graduation was 13 years; 48.4% were women, 94.2% worked in outpatient clinics, 29.0% had been recertified, 51.6 % had no training in musculoskeletal ultrasound and 35.5 % had an ultrasound machine at their workplace. Regarding perception, it was found that 99.4% of the participants consider that musculoskeletal ultrasound improves diagnostic accuracy, 97.4% think that it improves therapeutic effectiveness, 92.3% point out that it complements electrodiagnosis to identify anatomical variants and to establish a differential diagnosis, and 79.4% think that it improves patient satisfaction. In addition, 85.2% express high interest in training on the subject, 70.3% show high interest in its daily use and 49.0% consider its daily application viable.

Conclusions. The specialists in physical medicine and rehabilitation in Colombia have a high perception of the usefulness of musculoskeletal ultrasound in diagnostic precision, its effectiveness and as a safety guide in therapeutic procedures, as well as of patient satisfaction with this tool, which is in accordance with the available scientific evidence. Despite this, a quarter of these specialists perceived low viability to perform musculoskeletal ultrasound, which raises opportunities for research on barriers and facilitators and for cost-effectiveness studies, which will strengthen continued education and the use of musculoskeletal ultrasound in the rehabilitation services of the country.
